Targeted therapy extends progression-free survival of patients with advanced ovarian cancer
ScienceDaily (Dec. 29, 2011) ? Targeted drugs, which block or disrupt particular molecules involved in the growth of tumors, have been shown to be effective treatments against many types of cancer. A new phase 3 clinical trial conducted by the Gynecologic Oncology Group (GOG) showed that a targeted therapy called bevacizumab (Avastin) effectively delayed the progression of advanced ovarian cancer. Patients with newly diagnosed advanced ovarian cancer now typically undergo surgery and chemotherapy, but the new research suggests an additional avenue of treatment.
The results of the trial appear in the December 29 issue of the New England Journal of Medicine.
"This approach can be looked upon as a third major component of treatment for ovarian cancer and related malignancies," says Robert A. Burger, MD, lead investigator on the GOG study and director of the Women's Cancer Center at Fox Chase Cancer Center. "We've had the combination of surgical management and cytotoxic chemotherapy for many years, but we haven't really seen anything else in terms of a fundamental class of treatment. This represents a new way for us to control the disease."
The placebo-controlled study, which was sponsored by the National Cancer Institute, enrolled 1,873 patients with previously untreated advanced disease from 336 sites, primarily in the United States, but also in Canada, South Korea, and Japan. The patients either had stage III ovarian cancer that could not be entirely removed with surgery, or stage IV disease, and were randomly assigned to one of three groups. For patients who received bevacizumab with chemotherapy followed by bevacizumab for up to an additional 10 months, the median time until their cancer progressed was 14.1 months, compared to 10.3 months for patients in the control group, who received chemotherapy with a placebo and then continued with a placebo. The net effect was a 28% reduction in the risk of disease of ovarian cancer progression over time. Patients who received bevacizumab only with chemotherapy, but not afterward, had a median progression-free survival of 11.2 months.
The National Cancer Institute estimates that nearly 22,000 women were diagnosed with ovarian cancer in 2011, and more than 15,000 died of the disease. For patients diagnosed before the cancer has spread, the five-year relative survival rate is about 93 percent (relative survival measures survival of cancer only, independent of other causes of death). But ovarian cancer is insidious -- early symptoms, like bloating, abdominal pain, and trouble eating, are typical of many illnesses and easily dismissed as non-threatening. Women often do not learn they have the disease until it's already spread. In 62 percent of new cases, the patient's cancer has metastasized to distant sites, and the five-year survival rate is just under 27 percent.
Bevacizumab is already FDA-approved for use against some types of colon, lung, kidney and brain cancers; its accelerated approval for metastatic breast cancer was recently revoked by the FDA. The drug acts by binding with vascular endothelial growth factor (VEGF), a protein produced by certain cancers that helps initiate the growth of new blood vessels that feed the tumor. The process of growing new blood vessels is called angiogenesis, and bevacizumab is an angiogenesis inhibitor.
"Bevacizumab blocks the growth factor VEGF, which is important in the process of ovarian cancer progression," says Burger, "and we've seen that this drug is also active in patients with recurrent disease."
Angiogenesis happens at the interface between the host and the disease, which makes it an appealing target for treatment, says Burger, who also led the Phase II GOG study on using bevacizumab in women with recurrent ovarian cancer. He says different ovarian cancers may appear identical under the microscope but differ biologically, which means they'll respond differently to treatment.
In the NEJM paper, Burger and his co-authors point out that another ovarian cancer trial conducted primarily in Europe called ICON7 demonstrated positive results in using becavizumab in combination with chemotherapy and then continued for up to 7 months.

DES MOINES, Iowa (AP) ? At least $12.5 million and counting has blanketed the airwaves ahead of next Tuesday's Republican presidential caucuses, with hard-hitting commercials awash in ghoulish images and startling claims. Most are coming from a proliferation of new independent groups aligned with the candidates.
To hear the ads tell it, Newt Gingrich is a "serial hypocrite," Rick Perry "double dips" as governor and the "liberal Republican establishment" is plotting to anoint Mitt Romney as the party's presidential nominee. The attacks, the bulk of the commercials on the air, reflected the volatile state of the race five days before the first votes of the GOP presidential nominating contest.
After a slow start, the ads in Iowa are coming on fast and furious.
On Thursday alone, at least five new commercials were rolled out, including one by Perry castigating his rivals as Washington insiders and saying: "The fox guarding the henhouse is like asking a congressman to fix Washington: bad idea." An outside group aligned with Romney, Restore Our Future, rolled out a new spot that criticizes Gingrich and asks: "Haven't we had enough mistakes?"
In the final days of the Iowa campaign, most of the ads are deeply negative, thanks in large part to the proliferation of outside groups, known as super PACs, that are doing the dirty work for candidates they support. Gingrich has been the biggest target, withering under attacks from Ron Paul and Rick Perry's campaign as well as from several outside groups like the one aligned with Romney. Polls show that Gingrich's standing in Iowa has slid accordingly.
"I call it ad wars whack-a-mole ? this endless attacking in all directions, trying to slam down anyone who is surging to the top," said David Perlmutter, a University of Iowa journalism professor who studies political communication. "This is the most negative I've ever seen it. The ads are so blatantly negative I would have told you 10 years ago this would never fly in Iowa."
It's a different landscape in the campaign advertising world than four years ago when Barack Obama won Iowa's Democratic caucuses and Mike Huckabee carried the Republican side. Social media has intensified the advertising binge, with many spots debuting on TV but also going viral across the web at almost no cost to the campaigns that sponsor them. Candidates are making heavy use of online advertising to target voters based on location and other demographic information.
Campaigns are also producing video specifically for the YouTube audience, like a new 90-second Romney video excerpting a speech Obama delivered in Iowa days before winning the Democratic caucuses in 2008.
"Well, Mr. President, you've had your moment ... this is our time," Romney says in the spot.
On Thursday, Jon Huntsman's campaign ? which can't afford to put commercials on TV and is competing only in New Hampshire ? hit at Paul in a new web video that highlights comments about race and gays in newsletters Paul used to put out. The ad asks: "Can New Hampshire voters really trust Ron Paul?'"
But nothing has altered the environment more than super PACs, which are facing their first test in a presidential campaign since a Supreme Court decision two years ago eased restrictions on campaign spending by corporations, unions and individuals.
Much of $12.5 million spent to date in Iowa, a figure confirmed by ad tracking firms, outside groups and the GOP campaigns, has been spent in just the past few weeks, much of it paying for negative ads.
The pro-Romney super PAC, Restore Our Future, has been by far the most influential in Iowa, helping to bolster the former Massachusetts governor's position in the state he lost in 2008, crippling that campaign.
The group formed by Romney allies has spent at least $2.7 million in the state. The vast majority has been used to trash Gingrich, the former House speaker whose sudden surge in the polls earlier this month has been summarily halted in recent days. In ad after ad, Romney's allies have berated Gingrich for ethical "baggage," accepting $1.6 million in consulting fees from federal mortgage giant Freddie Mac and pledging to tackle climate change in an ad with House Democratic Leader Nancy Pelosi.
Another new ad from the group goes after both Gingrich and Perry for being "liberal on immigration."
Perry, the Texas governor, has defended his state's policy of allowing the children of illegal immigrants to pay in-state tuition rates at public universities, while Gingrich has spoken out against deporting those who have lived in the U.S. for many years without permission to be in the country.
The ad also chides Perry for taking advantage of a loophole in state law that allows him to supplement his governor's salary with his $90,000 annual pension, even as Perry has used his own ads to rail against congressional salaries.
Romney has stepped up his advertising presence in Iowa, driving a largely positive message while his allies have made it easy for him to avoid attacking his Republican rivals.
"In the campaign to come, the American ideals of economic freedom and opportunity need a clear and unapologetic defense. And I intend to make it because I have lived it," Romney says in a new uplifting 60-second commercial he began airing Thursday.
Gingrich, for his part, has railed against the Romney allies' ad blitz but has refused to respond in kind. A pro-Gingrich super PAC has begun fighting back, running ads in Iowa claiming the Republican establishment is "attacking him with falsehoods."
The ad warns: "Don't let the liberal Republican establishment pick our candidate."
But the assist from the pro-Gingrich group Winning Our Future may be too little, too late. A new CNN-Time poll found Gingrich now in fourth place in Iowa, behind Romney, Paul and Rick Santorum.
Other Republican hopefuls have super PACs that support them, including Perry and Santorum. The former Pennsylvania senator has run no ads of his own but has seen his position in Iowa strengthen in recent days in part by $327,000 in ad spending from a super PAC called the Red White and Blue Fund.
Our Destiny, a super PAC backing Huntsman, has run ads in New Hampshire for the former Utah governor. Huntsman is skipping the Iowa caucuses to focus on New Hampshire, which holds the nation's first primary on Jan. 10.
Perry has run the most campaign ads in Iowa, spending at least $3.9 million so far. His ads have offered a smattering of sometimes conflicting messages ? promoting his conservative Christian faith in one to calling for a part-time Congress in another.
"I'm an outsider who will overhaul Washington," Perry says in his latest ad, while pledging anew to end "Obama's war on religion."
Make Us Great Again, a pro-Perry super PAC, has also been on the air for weeks in Iowa.
The heavy spending hasn't seemed to help Perry much ? polls have consistently shown him trailing in the state, though he has gained some ground.
Paul has also been on the air for months and has not been shy about hitting his opponents. His latest ad, titled "Washington Machine," hits Gingrich as a "serial hypocrite" and Romney as a "flip flopper."
Cash strapped and struggling in polls, Michele Bachmann will run TV commercials a day before the caucuses. Her campaign has run radio ads and she's sought free media on a bus tour through Iowa's 99 counties.

WASHINGTON ? Starting in 2012, the government will charge a new fee to your health insurance plan for research to find out which drugs, medical procedures, tests and treatments work best. But what will Americans do with the answers?
The goal of the research, part of a little-known provision of President Barack Obama's health care law, is to answer such basic questions as whether that new prescription drug advertised on TV really works better than an old generic costing much less.
But in the politically charged environment surrounding health care, the idea of medical effectiveness research is eyed with suspicion. The insurance fee could be branded a tax and drawn into the vortex of election-year politics.
The Patient-Centered Outcomes Research Institute ? a quasi-governmental agency created by Congress to carry out the research ? has yet to commission a single head-to-head comparison, although its director is anxious to begin.
The government is already providing the institute with some funding: The $1-per-person insurance fee goes into effect in 2012. But the Treasury Department says it's not likely to be collected for another year, though insurers would still owe the money. The fee doubles to $2 per covered person in its second year and thereafter rises with inflation. The IRS is expected to issue guidance to insurers within the next six months.
"The more concerning thing is not the institute itself, but how the findings will be used in other areas," said Kathryn Nix, a policy analyst for the conservative Heritage Foundation think tank. "Will they be used to make coverage determinations?"
The institute's director, Dr. Joe Selby, said patients and doctors will make the decisions, not his organization. "We are not a policy-making body; our role is to make the evidence available," said Selby, a primary care physician and medical researcher,
But insurance industry representatives say they expect to use the research and work with employers to fine-tune workplace health plans. Employees and family members could be steered to hospitals and doctors who follow the most effective treatment methods. Patients going elsewhere could face higher copayments, similar to added charges they now pay for "non-preferred" drugs on their insurance plans.
Major insurers already are carrying out their own effectiveness research, but it lacks the credibility of government-sponsored studies.
Not long ago, so-called "comparative effectiveness" research enjoyed support from lawmakers in both parties. After all, much of the medical research that doctors and consumers rely on now is financed by drug companies and medical device manufacturers, who have a built-in interest in the findings. And a drug maker only has to show that a new medicine is more effective than a sugar pill ? not a competing medication ? to win government approval for marketing.
The 2009 economic stimulus bill included $1.1 billion for medical effectiveness research, mainly through the National Institutes of Health. It was not considered particularly controversial. But things changed during the congressional health care debate, after former GOP vice presidential candidate Sarah Palin made the claim, now widely debunked, that Obama and the Democrats were setting up "death panels" to ration care.
As a result, lawmakers hedged the new institute with caveats. It was set up as an independent nonprofit organization, with a .org Internet address instead of .gov. The government cannot dictate Selby's research agenda. And there are limitations on how the Health and Human Services department can use the research findings in decisions that affect Medicare and Medicaid.
Selby says the institute is taking seriously the term "patient-centered" in its name. Patients will not be merely subjects of research; they and their representatives will be involved in setting the agenda and overseeing the process.
"We are talking about patients as partners in the research," said Selby. Findings will be presented in clear language ? a kind of Consumer Reports approach ? so that patients and doctors can easily draw on them to make decisions.
"Our goal, our hope, is that over time, by involving patients in research, two things will happen," said Selby. "One is that we will start asking questions in a more practical fashion, so the results would speak more consistently to questions that patients want to know the answers to. And two is that, by our example of involving patients in the research, trust will rise." He expects to unveil the institute's proposed research agenda in the next few weeks.
Former Medicare administrator Gail Wilensky says that agenda should focus on high-cost procedures and drugs on which the medical community has not developed a consensus, and which have widely different patterns of use around the country. A Republican, Wilensky believes opposition to the institute's work is shortsighted.
"This just strikes me as a component of finding ways to treat better and spend smarter," she said.

Syphilis was one of the first global diseases, and understanding where it came from and how it spread may help us combat diseases today

In 1492, Columbus sailed the ocean blue, but when he returned from 'cross the seas, did he bring with him a new disease?
New skeletal evidence suggests Columbus and his crew not only introduced the Old World to the New World, but brought back syphilis as well, researchers say.
Syphilis is caused by Treponema pallidum bacteria, and is usually curable nowadays with antibiotics. Untreated, it can damage the heart, brain, eyes and bones; it can also be fatal.
The first known epidemic of syphilis occurred during the Renaissance in 1495. Initially its plague broke out among the army of Charles the VIII after the French king invaded Naples. It then proceeded to devastate Europe, said researcher George Armelagos, a skeletal biologist at Emory University in Atlanta.
"Syphilis has been around for 500 years," said researcher Molly Zuckerman at Mississippi State University. "People started debating where it came from shortly afterward, and they haven't stopped since. It was one of the first global diseases, and understanding where it came from and how it spread may help us combat diseases today."
Stigmatized disease
The fact that syphilis is a stigmatized sexually transmitted disease has added to the controversy over its origins. People often seem to want to blame some other country for it, said researcher Kristin Harper, an evolutionary biologist at Emory. [Top 10 Stigmatized Health Disorders]
Armelagos originally doubted the so-called Columbian theory for syphilis when he first heard about it decades ago. "I laughed at the idea that a small group of sailors brought back this disease that caused this major European epidemic," he recalled. Critics of the Columbian theory have proposed that syphilis had always bedeviled the Old World but simply had not been set apart from other rotting diseases such as leprosy until 1500 or so.
However, upon further investigation, Armelagos and his colleagues got a shock ? all of the available evidence they found supported the Columbian theory, findings they published in 1988. "It was a paradigm shift," Armelagos says. Then in 2008, genetic analysis by Armelagos and his collaborators of syphilis's family of bacteria lent further support to the theory.
Still, there have been reports of 50 skeletons from Europe dating back from before Columbus set sail that apparently showed the lesions of chronic syphilis. These seemed to be evidence that syphilis originated in the Old World and that Columbus was not to blame.
Armelagos and his colleagues took a closer look at all the data from these prior reports. They found most of the skeletal material didn't actually meet at least one of the standard diagnostic criteria for chronic syphilis, such as pitting on the skull, known as caries sicca, and pitting and swelling of the long bones.
"There's no really good evidence of a syphilis case before 1492 in Europe," Armelagos told LiveScience.
In the seafood?
The 16 reports that did meet the criteria for syphilis came from coastal regions where seafood was a large part of the diet. This seafood contains "old carbon" from deep, upwelling ocean waters. As such, they might fall prey to the so-called "marine reservoir effect" that can throw off radiocarbon dating of a skeleton by hundreds or even thousands of years. To adjust for this effect, the researchers figured out the amount of seafood these individuals ate when alive. Since our bodies constantly break down and rebuild our bones, measurements of bone-collagen protein can provide a record of diet.

LONDON (AP) ? Britain's Prince Philip returned to the royal family's country estate Tuesday, after a spell in the hospital undergoing treatment for a blocked coronary artery.
Philip, Queen Elizabeth II's 90-year-old husband, spent four nights in the hospital recovering from a successful coronary stent procedure. He was taken to Papworth, a specialist heart hospital in Cambridge, on Friday after complaining of chest pains.
It was the most serious health scare suffered by Philip, who is known to be active and robust. He has continued to appear at many engagements, most recently taking a 10-day tour of Australia with the queen.
For the first time in years he was forced to miss the royal family's traditional Christmas festivities, which include attending a morning church service, viewing the queen's annual Christmas broadcast together, and a shooting party on Boxing Day.
Philip did not speak to reporters as he was driven away from the hospital in a Range Rover Tuesday morning, though he smiled and waved to those gathered to film his departure.
"He is very much looking forward to rejoining his family," a Buckingham Palace statement said, adding that he also thanked the hospital staff for their care.
Philip will now return to Sandringham, the queen's huge private estate in rural Norfolk where the royal family retreats for the holiday season every year.
It is not yet clear if Philip's heart problem will cause a reduction in his plans to travel with the queen next year to celebrate her Diamond Jubilee. He is scheduled to make a series of trips to England, Scotland, Wales and Northern Ireland to help her mark her 60th year on the throne.
Dr. Allen Schwartz, chief of cardiology at New York-Presbyterian, Columbia University Medical Center, said that an otherwise healthy 90-year-old could be expected to resume normal activity shortly after having a stent procedure.
"The treatment now would be aimed at three things: recuperation from the procedure, which is usually relatively minor, eliminating factors that might lead to a recurrence of the problem, with an appropriate diet and exercise program, and, depending on the type of stent, some medicine to reduce the chance of clotting," Schwartz said.
Now that Philip has been found to have coronary artery blockages, he is likely to be treated with several medications that are routinely prescribed for heart patients.
In most cases these medicines would include a daily dosage of aspirin to thin the blood, a statin to lower cholesterol, and possibly a beta-blocker and a separate medicine to control blood pressure. Philip would also be expected to have his heart function tested every six months or so to check for any changes.
Philip had already announced when he turned 90 that he intended to slow down his extremely active schedule. The Diamond Jubilee plans reflected this desire, with the queen deciding to send her children and grandchildren on grueling overseas trips to Commonwealth countries while she and her husband make less demanding trips throughout the United Kingdom.

Report: Phishing attack targets Apple customers
A ?vast phishing attack? that attempts to capture the credit card information of Apple customers was launched on Christmas day, according to a report from Mac security-software company Intego.
The phishing email, as posted by Intego.In a posting on its Mac Security blog, Intego says that the attack is an attempt to fool Apple customers into clicking on a link under the guise of updating the billing information of their Apple accounts:
If you click on the link in the message, you will be taken to a realistic looking sign-in page, then, after entering your Apple ID and password, you?ll be taken to a page asking you to update your account profile, notably entering your credit card information. Again, this page looks realistic, and many of the elements it contains are taken from Apple?s own webpages.
Intego reports that the messages are being sent with the subject ?Apple update your Billing Information? from a spoofed email address of ?appleid@id.apple.com,? though of course future emails from the same source might vary somewhat.
If you hover your mouse over the hyperlink in the (impressively forged) email address, you?ll see a floating box that reveals the real destination of that link: the telltale chain of four numbers that specifies a numeric IP address, rather than a link to somewhere within the apple.com domain. As Intego rightly points out, ?if it?s not something.apple.com (it could be www.apple.com, store.apple.com, or something else), then it?s bogus.?
In addition to hovering your cursor over any links before you click on them, another way to stay secure is to enter links yourself in your browser rather than click on them in emails. If you type store.apple.com into your browser, you know it?s a legitimate site. If you?re using Safari any secure connection to Apple (i.e., any URL beginning with https: rather than http:) will show a green verification item in the top right corner of the address bar. (There are similar indications in other browsers.) And no legitimate site will ask for personal information, especially of the credit-card variety, without using a secure connection.This isn?t the first such scam posing as an email from Apple recently. In a less sophisticated attack earlier this month, a fake MobileMe message requested that users send an email containing their username and password.
In general, you should be skeptical about any email messages, however legitimate they appear to be, that ask you to go to a website or compose an email containing personal data.

SHELLSBURG, Iowa (AP) -- The mayor-elect of a small town in
eastern Iowa has died just days before she was set to take office.
The Gazette (http://bit.ly/uzCotW ) reports that 70-year-old
Joyce Pence died at her home in Shellsburg on Friday.
Cedar Memorial Park & Funeral Home confirmed the death Monday,
but said it had no information on the cause.
Pence was elected mayor for a two-year term in the November
election, and was going to take office during the city council's
next meeting in January.
She was a retired vice president at United Security Bank who
founded the Shellsburg Farmers Market in 2007.
A service celebrating her life is scheduled for Friday at the
Wildcat Golf Course Clubhouse.
Shellsburg is a town of 1,000 people about 20 miles west of
Cedar Rapids.

Chinese banks under pressure to raise cash next year: source (Reuters)
SHANGHAI (Reuters) ? China's listed banks are expected to raise over 100 billion yuan ($15.78 billion) through equity financing next year as they face pressure to replenish capital due to rapid loan growth and tighter regulation, the China Securities Journal reported on Tuesday.
Citing analysts, the official paper said banks facing such pressure included Agricultural Bank of China (601288.SS) (1288.HK), Industrial Bank Co Ltd (601166.SS) and Bank of Communications (601328.SS) (3328.HK).
Banks are also expected tap the debt market for funding, it said.
The bulk of the expected equity financing would be funded by government shareholders, the paper said. Listed banks raised more than 270 billion yuan via equity financing in 2010 and nearly 290 billion yuan worth of debt this year, according to the article.
Chinese banks will be under fundraising pressure because new lending next year is expected to exceed 8 trillion yuan -- a level necessary to support economic expansion -- potentially weakening lenders' balance sheets, the newspaper said.
Major Chinese banks, or systematically important ones, would be required to have minimum capital adequacy ratio (CAR) of 11.5 percent, while smaller banks must have at least 10.5 percent, according to rules published by the banking regulator in August.
However it is still unclear when they will be implemented.
China International Capital Corp estimates banks' assets to expand by around 14 percent next year and that would reduce the average CAR of listed domestic banks to around 10.7 percent, meaning some lenders would be under pressure to raise money, the paper said.

Anti-Putin protests draw tens of thousands (AP)
MOSCOW ? Tens of thousands of Russians jammed a Moscow avenue Saturday to demand free elections and an end to Prime Minister Vladimir Putin's 12-year rule, in the largest show of public outrage since the protests 20 years ago that brought down the Soviet Union. Gone was the political apathy of recent years as many shouted "We are the Power!"
The demonstration, bigger and better organized than a similar one two weeks ago, and smaller rallies across the country encouraged opposition leaders hoping to sustain a protest movement ignited by a fraud-tainted parliamentary election on Dec. 4.
The enthusiasm also cheered Mikhail Gorbachev, the last Soviet leader who closed down the Soviet Union on Dec. 25, 1991.
"I'm happy that I have lived to see the people waking up. This raises big hopes," the 80-year-old Gorbachev said on Ekho Moskvy radio.
He urged Putin to follow his example and give up power peacefully, saying Putin would be remembered for the positive things he did if he stepped down now. The former Soviet leader, who has grown increasingly critical of Putin, has little influence in Russia today.
But the protesters have no central leader and no candidate capable of posing a serious challenge to Putin, who intends to return to the presidency in a March vote.
Even at Saturday's rally, some of the speakers were jeered by the crowd. The various liberal, nationalist and leftist groups that took part appear united only by their desire to see "Russia without Putin," a popular chant.
Putin, who gave no public response to the protest Saturday, initially derided the demonstrators as paid agents of the West. He also said sarcastically that he thought the white ribbons they wore as an emblem were condoms. Putin has since come to take their protests more seriously, and in an effort to stem the anger he has offered a set of reforms to allow more political competition in future elections.
Kremlin-controlled television covered Saturday's rally, but gave no air time to Putin's harshest critics.
Estimates of the number of demonstrators ranged from the police figure of 30,000 to 120,000 offered by the organizers. Demonstrators packed much of a broad avenue, which has room for nearly 100,000 people, about 2.5 kilometers (some 1.5 miles) from the Kremlin, as the temperature dipped well below freezing.
A stage at the end of the avenue featured banners reading "Russia will be free" and "This election Is a farce." Heavy police cordons encircled the participants, who stood within metal barriers, and a police helicopter hovered overhead.
Alexei Navalny, a corruption-fighting lawyer and popular blogger, electrified the crowd when he took the stage. He soon had the protesters chanting "We are the power!"
Navalny spent 15 days in jail for leading a protest on Dec. 5 that unexpectedly drew more than 5,000 people and set off the chain of demonstrations.
Putin's United Russia party lost 25 percent of its seats in the election, but hung onto a majority in parliament through what independent observers said was widespread fraud. United Russia, seen as representing a corrupt bureaucracy, has become known as the party of crooks and thieves, a phrase coined by Navalny.
"We have enough people here to take the Kremlin," Navalny shouted to the crowd. "But we are peaceful people and we won't do that ? yet. But if these crooks and thieves keep cheating us, we will take what is ours."
Protest leaders expressed skepticism about Putin's promised political reforms.
"We don't trust him," opposition leader Boris Nemtsov told the rally, urging protesters to gather again after the long New Year's holidays to make sure the proposed changes are put into law.
He and other speakers called on the demonstrators to go to the polls in March to unseat Putin. "A thief must not sit in the Kremlin," Nemtsov said.
The protest leaders said they would keep up their push for a rerun of the parliamentary vote and punishment for election officials accused of fraud, while stressing the need to prevent fraud in the March presidential election.
Former world chess champion Garry Kasparov was among those who sought to give the protesters a sense of empowerment.
"There are so many of us here, and they (the government) are few," Kasparov said from the stage. "They are huddled up in fear behind police cordons."
The crowd was largely young, but included a sizable number of middle-aged and elderly people, some of whom limped slowly to the site on walkers and canes.
"We want to back those who are fighting for our rights," said 16-year-old Darya Andryukhina, who said she had also attended the previous rally.
"People have come here because they want respect," said Tamara Voronina, 54, who said she was proud that her three sons also had joined the protest.
Putin's comment about protesters wearing condoms only further infuriated them and inspired some creative responses. One protester Saturday held a picture montage of Putin with his head wrapped in a condom like a grandmother's headscarf. Many inflated condoms along with balloons.
The protests reflect a growing weariness with Putin, who was first elected president in 2000 and remained in charge after moving into the prime minister's seat in 2008. Brazen fraud in the parliamentary vote unexpectedly energized the middle class, which for years had been politically apathetic.
"No one has done more to bring so many people here than Putin, who managed to insult the whole country," said Viktor Shenderovich, a columnist and satirical writer.
Two rallies in St. Petersburg on Saturday drew a total of 4,000 people.
"I'm here because I'm tired of the government's lies," said Dmitry Dervenev, 47, a designer. "The prime minister insulted me personally when he said that people came to the rallies because they were paid by the U.S. State Department. I'm here because I'm a citizen of my country."
Putin accused the United States of encouraging and funding the protests to weaken Russia.
Putin's former finance minister surprised the protesters by saying the current parliament should approve the proposed electoral changes and then step down to allow new parliamentary elections to be held. Alexei Kudrin, who remains close to Putin, warned that the wave of protests could lead to violence and called for establishing a dialogue between the opposition and the government.
"Otherwise we will lose the chance for peaceful transformation," Kudrin said.
Kudrin also joined calls for the ouster of Central Election Commission chief Vladimir Churov.
Putin has promised to liberalize registration rules for opposition parties and restore the direct election of governors he abolished in 2004. Putin's stand-in as president, Dmitry Medvedev, spelled out those and other proposed changes in Thursday's state-of-the nation address.
Gorbachev, however, said the government appears confused.
"They don't know what to do," he said. "They are making attempts to get out of the trap they drove themselves into."

Rwandan rebel freed after intn'l charges dropped (AP)
THE HAGUE, Netherlands ? The International Criminal Court freed a Rwandan rebel Friday and returned him to France after dismissing murder and rape charges against him.
Callixte Mbarushimana, who spent 11 months in detention, was the first war crimes suspect to be arrested and freed without trial since the court began work in 2002 as the world's first permanent war crimes tribunal.
The court said Mbarushimana, who had lived in Paris before his arrest, was "released on French territory, as he requested" through the cooperation of the Netherlands and France.
Mbarushimana was charged with five counts of murder, rape, torture and persecution allegedly committed by Rwandan rebels in neighboring Congo.
In a 2-1 decision, pretrial judges ruled last week that the evidence against him was insufficient to warrant a trial, and ordered him freed.
Prosecutors lost an appeal last Tuesday, but his release was delayed because he was still the subject of a U.N. travel ban. France asked for a delay to allow time to clear his transfer with the U.N. Sanctions Committee.
Prosecutors accused Mbarushimana of being a senior member of the Democratic Forces for the Liberation of Rwanda, known by its French acronym FDLR, which is made up of former guerrillas accused of genocide in Rwanda's 1994 ethnic slaughter.
The group's killing spree in Congo in 2009 that left hundreds dead was aimed at forcing Rwanda to accept their return.
Prosecutors allege that Mbarushimana's role was coordinating "an international campaign of propaganda and extortion" aimed at furthering the FDLR cause.
Mbarushimana signed his name to FDLR press releases issued from Paris, but denied his group was involved in atrocities. He described the FDLR as a political and military organization intent on bringing reform to Rwanda.
It was yet another narrow escape for Mbarushimana, a former computer technician for the U.N. Development Program, who has evaded trial several times for the 1994 genocide and the ensuing struggle. Some half a million Tutsis were murdered by ethnic Hutus in 100 days in the summer of 1994.
Mbarushimana was arrested and held for two months in 2001while working for the U.N. in Kosovo before he was released because his indictment was faulty. Later he was indicted by the International Criminal Court for Rwanda, but again his case was dropped, apparently because the court was focusing on higher-level suspects. In 2008 he was arrested in Frankfurt when German authorities found his name on the U.N. sanctions list. He was released four months later.

Urijah Faber shaves his famous locks to support sister
One of Urijah Faber's trademarks, his golden brown hair, is now gone after he shaved his head to support his ailing sister.
The day after Thanksgiving, his 19-year-old sister was in a serious car accident. Michaela has been in the hospital since then, and had brain surgery requiring a shaved head. Faber, who will soon begin coaching on "The Ultimate Fighter" against bantamweight champion Dominick Cruz, has been by his sister's side through much of her trauma.
He tweeted before the haircut that it's been 12 years since he had it that short, but was happy because Michaella is coming home on Friday. Because of his new haircut, she won't be the only person at family holiday celebrations without any hair.
